Today at Givens Highland Farms, residents and future residents gathered for an engaging and informative session, Care Across Life Transitions: Understanding the Care Continuum—Who’s Here to Support You.
We’re incredibly grateful to Nick for coordinating this event and helping bring together such a meaningful conversation. Thank you as well to all who attended—your participation, questions, and insights made the session both impactful and relevant.
The interdisciplinary Resident Resource Team led attendees through a series of real-life scenarios, offering a clearer picture of how support evolves across different stages of life and care needs. These discussions are an important part of ensuring residents feel informed, empowered, and supported—no matter what the future holds.
We’re especially proud to have had Lisa Constantine, Director of Home Care, and Julie Moses, Aging in Place Director, represent Givens Home First on the panel. Their expertise and compassionate approach helped illustrate how our teams walk alongside residents through every transition.
At Givens, we believe that understanding your options—and knowing who is there to support you—can make all the difference. Today’s session was a powerful reminder of the strength of our community and the value of coming together to learn, ask questions, and plan ahead.
