With a personalized approach, we’ll work closely with residents of our two Life Plan Communities, their families, social workers and healthcare providers to ensure seamless access to the continuum of care. We embrace open and candid conversations about care levels, with a Community Nurse that’s here to guide residents through discussions with information, support and empathy. Whether it’s connecting with specialized care providers or offering guidance on available resources, Community Nursing is designed to help our residents maintain their well-being and independence within the nurturing environment of Givens Estates and Givens Highland Farms.
